The Library still has a limited amount of 2025 State and Federal Tax Forms available for pick up behind the Second Floor Information Desk.  This limited quantity of Tax forms is available for pick up free of charge.  Available forms include:

State Tax Forms

  • Form 1: Wisconsin Income Tax
  • Form 1 Instructions
  • Schedule WD: Capital Gains & Losses
  • Schedule WD Instructions
  • H & H-EZ Schedules and Instructions: Wisconsin Homestead Credit

Federal Tax Forms

  • 1040
  • 1040-SR
  • 1040 Schedule 1-A: Additional Deductions
  • 1040 & 1040-SR Instructions Booklets

Additional Tax Forms

Tax forms that are not available at the Second Floor Information Desk can still be printed out for the price of .15 cents for black and white and .50 cents for color per page.  If assistance is needed, a staff member at the desk would be happy to help.
